This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6. SET-ASIDE for small business Indian firms under the Buy Indian Act.

This Request for Proposal is for the following non-personal Registered Nurse (RN) services; Case Manager, Nurse Educator, and Emergency Room. The services are to be performed at the Gallup Indian Medical Center (GIMC) health care facility located in Gallup, New Mexico.

Nurse Educator (Outpatient/Inpatient) - 5 Total Nurse Case Managers (Outpatient/Inpatient) - 6 Total Emergency Room - 13 Total

The tasks to be performed are provided in the Performance Work Statement (PWS) and the associated Position Description (PD).

The proposed services are to begin within three to four weeks (contingent upon selection and pre-security background clearance) due to urgent requirements. The projected performance start dates are 12 or 19 March 2018 for 30 weeks. Option to extend to 52 weeks may be offered if the position has not been permanently filled.

a. Complete Candidate Profile shall include using the attached checklist. Only complete profiles shall be accepted.

b. PRICING SCHEDULE: The proposed rate shall be an All-Inclusive Bill Rate. Rates will be negotiated upon selection.

c. Three company past performance references of similar scope (not the RN references): Limit one page for each reference.

d. Submission date will close 12 March 2018, 3 PM (MST) by email.

e. Submit questions by 1 March 2018, 12 PM (MST).

The Government reserves the right to award to a single vendor or multiple vendors. Awards may be established prior to the RFP closing date of 12 March 2018. Offerors are not required to submit a quote for all line items listed on the pricing schedule to be considered.

The contractor will be responsible for filling vacancies longer than two weeks duration for their awarded positions. In determining best value, the following factors will be considered: Price, Past Performance (Experience, References, and Qualifications) of each proposed nurse candidate. As part of the evaluation process, nurse candidates shall be interviewed to ensure their understanding of the requirements and to verify qualifications. All evaluation factors other than cost or price, when combined, are significantly more important than price. A responsibility determination will be made of all awardees in accordance with FAR Part 9.103.
