Quillen VA Medical Center is seeking court reporter services.All interested companies shall provide quotations for the following:Statement of WorkReference No: 621-21-1-4022-0022Service: Human ResourcesService Period: January 26-28, 2021Title of Project: Court reporter services for a Disciplinary Action Board (DAB) hearing scheduled for January 26-28, 2021, for James H. Quillen VA Medical Center (JHQVAMC). Scope of Work: The court reporter will serve to obtain accurate transcripts of witness testimony during the hearing. The reporting firm will supply any needed equipment to obtain the testimony. Due to the statutory deadline imposed for the completion of a DAB process, the reporting firm will provide the official, certified transcripts, electronically only, within one week of the completion of witness testimony. Original transcript recordings are the property of the JHQVAMC and should be returned or a certificate of destruction provided. The court reporter will be at JHQVAMC in a private conference room which is large enough to accommodate social distancing. The conference room is in the Human Resources (HR) department, room 154, first floor of building 8 on Dogwood Drive, Mountain Home, TN. JHQVAMC does not have a physical address (P.O. Box only). Driving instructions can be provided if needed. Reserved parking for HR is located in front of our building on Dogwood Drive. Background: The board members will conduct this hearing virtually. The court reporter, witnesses and agency representative will be on site at JHQVAMC. Performance Period: Testimony will begin at 9:00 am each morning and expect to go until 5:00 pm on January 26, 2021, and continuing as needed January 27-28, 2021. An hour lunch and 15-minute rest breaks are planned. The court reporter will be required to wear a face mask while on site and will be health-screened each morning as they arrive (just inside the entrance to the facility while still in their vehicle). In order to start on time, the court reporter should plan to arrive at least 30-45 minutes before the beginning of the hearing for the health screening and equipment set up each morning.Services: The contractor shall provide the following services: a. Provide necessary equipment in order to transcribe proceedings of the hearing for each day witnesses are scheduled, and; b. Provide an electronic copy only of the official, certified transcripts within one week of the completion of the witness testimony. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
