The Network 10 Contracting Office, PO Box 492, Ann Arbor, Michigan, 48106, is soliciting - RFQ to be posted to FBO on or about March 16, 2018 - and intends to award a firm fixed price, IDIQ contract for Nursing Home services for beneficiaries of the Ann Arbor VAMC, 2215 Fuller Rd., Ann Arbor, MI. 48105. NAICS Code 623110, size standard $27.5 million. The contractor shall be located in the counties of Washtenaw or Jackson, in the State of Michigan-USA. This solicitation shall be conducted in accordance with FAR 13.5 and VAAR 873. The Contractor shall provide skilled nursing home services for veterans. The period of performance is five (5) one (1) year ordering periods from June 1, 2018 through May 31, 2023, in strict accordance with all schedules, specifications, terms, conditions and provisions of the contract solicitation. The Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the SAM database to ensure it is current, accurate and complete. Contract must also provide copies of current Certificate of Insurance as required by VAAR 852.237-7; and copy of most current Medicaid Rate Letter.
