This document is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested, and a written solicitation will not be issued.

The proposed contract action is for supplies or services for which the Government intends to solicit Resonance Technology, Inc. Interested persons may submit offers, and all eligible offers received prior to May 29, 2015 at 5:00 PM PDT will be considered by the Government. The NAICS code is 621512 and the size standard is $15 Million. This is an open market competitive acquisition.

This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-82 effective, September 03, 2013.

FAR 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price

and other factors considered. The following factor shall be used to evaluate offers:

The Government will award to the lowest price, technically acceptable offeror. Brand name only.

(b) Options. The Government will not evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

1) Essential/significant physical, functional, or performance characteristics.

a) Compatibility - The system must be compatible with the VA NCHCS computer system: PACS

System (Philips iSite 4.4) and DICOM 3.0; Patient intercom system

b) Dimensions - Approximately 50 feet in length

c) Weight- N/A

d) Industry Standard - This SPECT system is required to meet the imaging needs of nuclear medicine services.

e) Purpose - SPECT gamma camera for all non-PET nuclear imaging studies to be used while construction and installation of new equipment and acceptance by medical physicist is underway.

f) Governing Body or Organization - Not Required.

2) Complete generic identification - Not Required.

3) Applicable model/make/catalog number - Mobile Siemens dual head SPECT unit with associated computer systems having eSoft version 3.0.7.32 installed. Prefer 4DM cardiac software.

4) Manufacturer name - Alliance Imaging's Mobile SP 20 Unit with a Dual Head Siemens' Ecam

Statement of Work

Project Title: Nuclear Medicine Dual Head SPECT Mobile Camera

Background: VA NCHCS Nuclear Medicine Department at Mather is replacing existing gamma cameras with two SPECT/CT systems. Construction and installation will take approximately four months. To insure continual care for our Veterans, we will need to have a portable dual head SPECT camera while the project is ongoing. A trailer and camera are to be placed immediately outside of the VA NCHCS hospital. The trailer should have room for wheelchair access.

Scope of Work: The contractor shall provide a Siemens ECam dual headed SPECT camera and trailer. Contractor's labor responsible for installation, confirmation of imaging quality, power, phone and data connections and 3 days of Applications training. Contractor is responsible for delivery, installation, set up; contractor must provide most current health physics certification at time delivery. Contractor is responsible for maintenance and repairs of the trailer, maintenance and repairs of the SPECT system and all items from bumper to bumper. Hydraulic Wheelchair lift must be fully operational and tested at time of delivery; Patient safety rails, stairs and railings; SPECT equipment must be fully compatible with existing hospital PACS System( Philips iSite 4.4) and DICOM 3.0; Patient intercom system must be tested and operational at time of delivery; All safety and warning systems both visual and audio must be tested and operational at time of delivery; SMO Fire Extinguishers with current date of expiration; all phone and data ports must be tested and operational; contractor must include 480v 50 foot power cable or longer with necessary attachments to our existing infrastructure. On board generator to power HVAC must be tested and operational at time of delivery; all necessary operators' manuals must be delivered. Full System service available M-F with onsite service support.

SPECT Technical requirements; The following are the necessary technical requirements:

" (2) 3/8" HD3 high definition digital detectors

" Wholebody SPECT

" Dynamic SPECT

" Digital acquisition controller

" Dual 800 mHz Pentium 3 cpu's

" 18 GB scsi disk drive

" High resolution monitor

" Automatic image reconstruction

" Patient position monitor

" biCORE rapid insert collimator exchange system

" ecam processing software

" esoft archival, CD-R

" remote access kit for e soft

" ECG gate w/out strip chart

" Ecam patient bed

" Medium energy, low penetration collimators

" Low energy high resolution collimators

" Collimator cart

" Cardiac software, prefer 4DM SPECT cardiac software

" DICOM file transfer and exchange

" Ability to remove Hard drive on termination of lease to ensure patient confidentiality

Performance Period: 4 months with option for early termination of contract if no longer needed. Contract anticipated to begin approximately June 12,2015. During performance period, contractor must maintain health physics compliance and meet necessary regulatory inspections of all federal agencies including NHPP, NRC and VA.

Type of Contract: Sole Source due to urgent need for patient care; firm fixed price with option of 30 day notice for early end of contract.
