Aetna Introduces Medicare Advantage Plan with Uniontown Hospital

UNIONTOWN, Pa.--(BUSINESS WIRE)-- Aetna (NYSE: AET) today announced a new Medicare Advantage plan with a low monthly premium and robust benefits for Medicare beneficiaries in Fayette County. The Advantra Fayette Health Prime Plan is a Medicare Advantage HMO based on Uniontown Hospital’s provider network, its affiliated physicians and other health care providers.

Plan highlights include a $5 copay for primary care physician office visits, a no-cost fitness membership to multiple gyms and more.

Medicare Advantage members enrolled in the plan who receive care through the Uniontown Hospital’s network and affiliated providers will experience more coordinated care. With the new plan, Aetna and treating physicians can better share information, which is particularly beneficial for members with chronic or complex conditions. Aetna nurse case managers will work with providers to assist in care coordination, outreach and follow-up services.

This plan will be available in Fayette County during Medicare Annual Enrollment period this fall with an effective date of January 1, 2016.
