This is a SOURCES SOUGHT ANNOUNCEMENT ONLY.Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Large and Small Businesses interested and capable of performing the work. Responses to this notice shall include the following:
(a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and (g) a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below.
All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov.
The qualifications of such personnel shall be subject to review by the VA Chief of Staff and approval by the VA facility Director. Personnel assigned by the contractor must have graduated from an accredited JRCERT (
(1) Copy of ARRT certification and current California CRT license.
(2) Fluoroscopy and BLS certificates (if applicable)
(3) Copy of insurance certificate, evidencing worker's compensation and medical liability coverage
It is the VA's intention to solicit proposals on a competitive basis for a Firm-Fixed Price contract. Proposed prices shall be all inclusive and will include technician fees, malpractice insurance, benefits and contractor's fees. The contract will be awarded as a base year (one year from date of award), with four (4) one-year renewal options that can be exercised at the option of the Government. This solicitation will be issued as a commercial item in accordance with FAR Part 12 and FAR Part 15. NAICS is 561320. The solicitation is expected to be issued on or about
