IT IS, THEREFORE, ORDERED as follows:

2. That, prior to resuming operations, the above-named motor carrier, that is the insured, shall provide proof of insurance. Said insurance must be issued by an insurance company authorized to do business in

3. That the above-named motor carrier shall notify the Commission by letter in the event that it: (a) desires to resume its operations under its above-noted certificate number at a time prior to the authorized suspension end date,
