STATEMENT OF WORK
Pharmacy Service Wewoka Indian Health Center
BACKGROUND AND PURPOSE OF THE CONTRACT
A full time contract pharmacist (contractor) and/or licensed pharmacist(s) employed by the contractor, whose mission is to provide accessible, full-scope, quality pharmacy services to eligible Indian patients at the
Pharmacy services will be available to eligible patients at the WIHC. The staff of the WIHC and the pharmacist(s) shall work in close cooperation in order to meet the needs of the patient.
The primary purpose of this contract is to provide the patients seen at the WIHC, quality pharmacy services. The Contractor(s) shall hold a current state license in pharmacy.

STATEMENT OF WORK
Pharmacy services shall be provided at the WIHC. Onsite services shall be available daily and scheduled in a manner mutually agreed upon by the contractor(s) and the service unit.
Clinical services: The services will include the following:
1. Assure appropriateness of drug therapy. 2. Verify that patients understand their medications and appropriate outcomes of their drug therapy. 3. Assure availability, preparation and control of medications. 4. Provide drug information, drug therapy consultation and staff education relating to drug therapy. 5. Provide health promotion and disease prevention activities relating to drug use and preventive drug therapy. 6. Manage therapy for selected patients in whom drugs are the primary method of treatment. 7. Performance of other assigned duties or agency-specific activities including administrative duties. 8. Performs a variety of professional tasks of average technical difficulty in fulfillment of the department's basic patient care and service functions. 9. Exercises independent judgment in accomplishment of a variety of professional tasks. Judgmental failure could seriously affect the health status of one or more individuals. 10. Normally exercises no supervision over other professionals. May from time to time supervise a support person in the accomplishment of individual tasks, but does not have overall supervisory authority for the support personnel. 11. Procedures for performing routine aspects of the work have been developed, and a variety of guidelines are available. Situations not covered are referred to a senior, more experienced pharmacist. 12. The Contractor shall provide Pharmacy Services and/or additional licensed Pharmacists, as needed, on a five (5) days per week per Contracted employee, not to exceed forty (40) hours per week, per Contractor. Overtime is not authorized. Services shall be scheduled in a manner that is mutually agreed upon between the Contractor and the service unit. This is a non-personal service fixed price contract. EVALUATION AND REPORTING REQUIREMENTS
Allow the government to evaluate the quality of professional and administrative services provided, but retain no control over the medical, professional aspects of the services rendered (e.g., professional judgement).
The service unit shall provide an ongoing method of monitoring and evaluating patient services set forth in this contract.
