This is a combined synopsis/solicitation for commercial items. This announcement constitutes the only solicitation; are being requested, and a written solicitation document will not be issued. This solicitation is a "request for quotations (RFQ). The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 (Surgical and Medical Instrument Manufacturing).

VA New Jersey Healthcare System East Orange Campus at 385 Tremont Avenue, East Orange, NJ and the VA New Jersey Healthcare System, Lyons Campus, 151 Knollcroft Rd., Lyons, NJ are seeking to purchase (Exact Match Only) Maxi Move and Sara 3000 Lifts. The contractor shall provide Low Castor Maxi Move Battery Powered Patient Lift equipment and SARA 3000 Battery Powered Patient sit to stand lift equipment for safe assisted transfer, positioning, & movement of dependent patients and residents for the sole purpose of patient care to the VA New Jersey Health Care East Orange & Lyons Campus Community Living Center (CLC). STATEMENT OF WORK

Low Castor Maxi Move Powered Patient Lift (Hoyer Lift)

SARA 3000

SCOPE OF WORK: The contractor shall provide Low Castor Maxi Move Battery Powered Patient Lift

equipment and SARA 3000 Battery Powered Patient sit to stand Lift equipment for safe assisted

transfer, positioning, & movement of dependent patients and residents for the sole purpose of patient

care to the VA New Jersey Health Care East Orange & Lyons Campus Community Living Center (CLC).

Equipment to be delivered at Quantity (1) each Maxi Move & SARA 151 Knollcroft Road Lyons, NJ 07939

& Quantity (2) each Maxi Move & SARA 385 Tremont Avenue, East Orange NJ 07018

PERFORMANCE PERIOD: This contract shall be for one-time purchase only under a firm fixed-price

contract.

BACKGROUND: Patient height, weight, and body shape are important to consider if a patient cannot

reposition or transfer self independently. Patient handling equipment and devices (Slings) come in

different sizes to accommodate patients of all shapes, sizes, thus helping to ensure safety of patients and

staff during transfer and movement. The goal of using Hoyer Lift is to minimize potential risk of injury to

patient and employees. The VANJHCS purchased (2) Maxi Move Lift Systems w/Scale and with supporting

Sling supplies in 2011 & 2018 from vendor ARJOHUNTLEIGH for its Nursing Home facility. Due to increase

in acuity and dependent patients, purchase of Maxi Move Hoyer Lift is highly important to reduce pt. and

staff potential for injury exposure. Maxi Move need to be purchased for ward 5B, Ambulatory Clinic in EO,

and CLC. New patient lift equipment is required to assist the staff with the daily management of patient care.

In regard to patient and employee safety as well as equipment uniformity; the facility is requesting exact

match only, since there is already Maxi Move equipment in use on the wards; which are considered to be a reliable and trusted patient lift system. In addition, the supporting slings supplies are the required slings to operate this equipment and the rest of the ARJO Maxi Move already in use at the VANJHCS. Exact Match Only.

MAXI MOVE: Maxi Move Hoyer Lift provides safe, comfortable handling of residents/patients by providing more ergonomic working environment for caregivers thereby reducing the risk of patient falls and musculoskeletal injuries of workers. The battery powered passive sling lift allows caregiver to safely manage a wide range of resident/patient handling routines. Maxi Move is used to lift and reposition patients. The handheld control unit enables the caregiver to provide a higher quality of patient care, the best available maneuverability for easy positioning in patient transfer routines. It allows the nurse to work in an ergonomically correct posture, to prevent static load injury.

Where will the equipment being requested be used: In direct patient care area units, wards, clinics, & Community Living Center that are at high risk for patient falls and staff injuries related to patient movement and care. Clinics, Wards, and CLC Resident rooms, bathrooms, on the hallways, and pick residents up from the floor after falls

Who will be using the equipment:

Direct patient care staff who have been trained through in-services to utilize the product (Primarily RN's, LPN s, NA's, CLC staff)

Delivery:

Equipment will be delivered to receiving department Lyons & East Orange campus warehouses. Operator Manuals to be provided to users and Service maintenance to be provided as needed.

Maintenance:

Vendor will provide manufacturer s cleaning instructions, and insure all property is cleaned and decontaminated prior to bringing to VANJHCS Lyons Campus.

Training / in-service:

Vendor MUST provide Unlimited Operator Training to all clinical staff using the equipment on proper use of equipment upon delivering of equipment 24/7, train all staff on different tours/shifts, and upon request five days a week Monday through Friday excluding holidays.

Award shall be made to the offeror whose quotation offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the Exact Match .

All offerors shall submit the following: Quote with DUNS number, letter of authorization, solicitation number, phone number, fax number, point of contact, SDVOSB VIP certification and proposed delivery date for goods. All offers shall be sent to the Contracting Office, Attn: [email protected].

This is an open-market combined synopsis/solicitation for Maxi Move and Sara 3000 Lifts as outlined above.----The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. Submission shall be received not later than 09/24/2018 10:00A.M. ONLY faxed or e-mailed quotes will be accepted.
