Wisconsin releases enforcement actions for June

The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for June.

The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.

Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Jacob L. Acton, 2413 Rutledge Ave., Janesville, WI 53545, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jaylah Askew, 15511 Jessie Parker Rd., Sugar Land, TX 77498-1845, had her non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due, failing to respond to OCI inquiries, and failing to participate as ordered in an administrative hearing.

Olivia Buenrostro, 4811 S. 76th St., Ste. 9, Greenfield, WI 53220-4364, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Julius L. Green, 7728 W. Heather Ave., Milwaukee, WI 53223-2504, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Joshua Grubb, 630 S. 5th Ave., Wausau, WI 54401, had his application for an insurance intermediary license denied. This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license.

Frank Guzman, 10640 NW 39th St., Coral Springs, FL 33065-2381, had his non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due, failing to respond to OCI inquiries, and failing to participate as ordered in an administrative hearing.

James Kesl, 407 Indian Blanket Dr., League City, TX 77573-7269, had his Wisconsin non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to pay a forfeiture when due, failing to respond to OCI inquiries, and failing to participate as ordered in an administrative hearing.

Kenneth Keusch, 79 Valley Forge Rd., Weston, CT 06883, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to disclose administrative actions taken by other states and violating a warning letter issued by OCI.

Damien King, 7819 Geoffrey Ct., Charlotte, NC 28213, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose administrative actions taken by other states and having criminal convictions substantially related to the circumstances of holding an insurance license.

Amanda Lee, 706 S. Spring St., Beaver Dam, WI 53916, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Joshua Mallen, 1100 Park Central Blvd. S., Ste. 2450, Pompano Beach, FL 33064, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to disclose administrative actions taken by other states and violating a warning letter issued by OCI.

Tamatha McCants, 2951 N. 49th St., Milwaukee, WI 53210, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Brianna Morales, 1975 E. Sunrise Blvd., Ste. 808, Fort Lauderdale, FL 33304, had her application for an insurance intermediary license denied. This action was taken based on allegations of having administrative actions taken by other states.

Long Moua, 8000 W. Brentwood Ave., Milwaukee, WI 53223-5527, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Melvin Muchnik, 67 N. Maplemere Rd., Amherst, NY 14221, had his non-resident insurance intermediary license revoked. This action was taken based on allegations of lacking the character required of insurance intermediaries.

Maxwell Nicholson, 11010 Burdette St., Omaha, NE 68164-3889, agreed to a voluntary surrender of his public adjuster registration. This action was taken based on allegations of failing to maintain a valid public adjuster bond and failing to respond to OCI inquiries.

Joseph Nickerson, 1762 Daystar Dr., Haines City, FL 33844-6140, had his non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to respond to OCI inquiries and failing to participate as ordered in an administrative hearing.

Ashley Okereke, 777 E. 15th St., Apt. 439, Edmond, OK 73013, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to timely report five (5) administrative actions taken by other states.

Dante Paone, 1419 NE 14th Ct., Ft. Lauderdale, FL 33304-1341, agreed to voluntarily surrender his Wisconsin insurance intermediary license for a period of five (5) years and pay a forfeiture of $1,500.00 upon reapplication. These actions were taken based on allegations of failing to pay a previous forfeiture when due.

Gregory Quinn, 290 King of Prussia Rd., Ste. 314, Radnor, PA 19087, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Magdala Sims, 10605 Boyette Creek Blvd., Riverview, FL 33569, agreed to a probationary non-resident insurance intermediary license for one year. This action was taken based on allegations of failing to disclose administrative actions taken by other states against an occupational license.

Coleen Smith, 4902 Pier Dr., Greenacres, FL 33463, was ordered to pay a forfeiture of $1,000.00 and had her non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to respond promptly to OCI inquiries and lacking the character required of insurance intermediaries.

Brian L. Weisbrot, 5718 NW 73rd Ave., Tamarac, FL 33321, was ordered to pay a forfeiture of $6,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product and failing to timely respond to OCI.

Christopher C. Wiltfang, P.O. Box 704, Darien, GA 31305, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Allegations and Actions Against Companies

Covenant Administrators LLC, 2810 Premiere Pkwy., Ste. 400, Duluth, GA 30097, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely disclose administrative actions taken by other states and violating a warning letter issued by OCI.

Allegations and Actions Against Other Entities

Robyn Bundy, 808 School St., Withee, WI 54498, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

LaToya Crawford, 4012 W. Goodhope Rd., Apt. A, Milwaukee, WI 53209, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Alen Jeske, 230 S. Webster Ave., Omro, WI 54963, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Sam Nelson, 1004 Townline Ave., Beloit, WI 53511, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
