Wisconsin releases enforcement actions for July
The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for July.
The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.
Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.
Allegations and Actions Against Agents
Nicolas J. Berken, N870 Julius Dr., Appleton, WI 54914, agreed to the suspension of his Wisconsin insurance intermediary license. This action was taken based on allegations of misrepresentation and sales of unsuitable health insurance.
Jonathan Clemens, 1257 Brayden Ln., Unit 38, De Pere, WI 54115, had his application for an insurance intermediary license denied. This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license and providing incomplete information on a license application.
Kevion Dean-White, 9560 N. 60th St., Brown Deer, WI 53223, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose pending criminal matters and a previous administrative action.
Michael J. Geer, 4830 N. Sourapple Dr., Appleton, WI 54913, agreed to the suspension of his Wisconsin insurance intermediary license. This action was taken based on allegations of misrepresentation and sales of unsuitable health insurance.
Francis Gordillo, 1116 S. Pecan Grove Ave., Gonzales, LA 70737-7289, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Shanna Grey, 2630 W. Broward Blvd., Ste. 203113, Ft. Lauderdale, FL 33312-1314, agreed to voluntarily surrender her Wisconsin insurance intermediary license. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Zoe Gruenberg, 53 Howard St., Apt. 4C, New York, NY 10013, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Daniel Herlehy, 1050 E. Perrin Ave., Ste. 107, Fresno, CA 93720, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state and violating a warning letter issued by OCI.
Jontai Holmes, 12750 Citrus Park Ln., Ste. 110, Tampa, FL 33625-3784, agreed to the voluntary surrender of his Wisconsin insurance intermediary license. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Nicole A. Houle, 270 Wintergreen Dr., Apt. 1, Omro, WI 54963, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
James P. Huskin IV, 1 Nationwide Plz., Columbus, OH 43215, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Miguel A. Ibarra, 2010 16th St., North Chicago, IL 60064, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Monica Klempel, 1515 Monroe Ave., River Forest, IL 60305, had her Wisconsin insurance intermediary license summarily suspended. This action was taken based on allegations of making misrepresentations to consumers regarding insurance products, making misrepresentations on insurance applications, offering benefits not specified in an insurance policy, and failing to respond to inquiries from OCI.
Joshua R. Knutson, 7906 Indigo Ridge Terr., Bradenton, FL 34201, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of making a misrepresentation in the sale of an insurance product and soliciting insurance on behalf of an insurer without an appointment with that insurer.
Faith M. Kristof, 932 Maple Hill Rd., Wausau, WI 54403-2237, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Rita Kumi, 440 Whitby Terr., Hampton, GA 30228, agreed to permanently surrender her Wisconsin insurance intermediary license. This action was taken based on allegations of submitting fraudulent insurance applications.
Angel M. Lahoz, 10117 Umberland Pl., Boca Raton, FL 33428, was ordered to pay a forfeiture of $18,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product and failing to respond to OCI's requests for information.
Donnie E. Leverette, Jr., 175 Berkeley St., Boston, MA 02116, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Estephania Lopez, 640 NW 79th Ave., Apt. 103, Pembroke Pines, FL 33024-6897, was ordered to pay a forfeiture of $2,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product and recommending the purchase of an unsuitable health insurance plan.
Laura A. Martinez, 1704 W. Calle Cedro, Edinburg, TX 78541-9664, had her insurance intermediary license revoked. This action was taken based on allegations of providing incorrect and misleading information on a license application and violating Wisconsin insurance law.
Santiago M. Mastoridis, 259 S. Cypress Rd., Apt. 545, Pompano, FL 33060, was ordered to pay a forfeiture of $2,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product and recommending the purchase of an unsuitable health insurance plan.
Shirley A. McFarlane, 8261 N. 48th St., Milwaukee, WI 53223-3705, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of making an unsuitable health insurance recommendation.
Robert Ochoa IV, 1311 Zureiq Ct., Clermont, FL 34714, had his application for an insurance license denied. This action was taken based on allegations of having a criminal conviction that may be substantially related to the circumstances of holding an insurance license.
Justin D. Paraison, 2704 NW 79th Ave., Margate, FL 33063-8151, was ordered to pay a forfeiture of $3,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product and recommending the purchase of an unsuitable health insurance plan.
Joseph V. Pettas, 825 SE 8th Ave., Ste. 204, Deerfield Beach, FL 33441-5645, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product.
Christopher Pierce, 1642 Bainbridge St., Apt. 28, La Crosse, WI 54603, had his application for a Wisconsin insurance intermediary license denied. This action was taken based on allegations of owing delinquent child support and having a criminal conviction that may be substantially related to insurance marketing type conduct.
Jirran Rainey, 3006 Madison Elm St., Katy, TX 77493, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to disclose an administrative action taken by another state and violating a warning letter issued by OCI.
Christine Ringmeier, 11823 Meyer Rd., Two Rivers, WI 54241, had her application for a Wisconsin insurance intermediary license denied. This action was taken based on allegations of having a criminal conviction substantially related to the circumstances of holding an insurance license, failing to report criminal charges, and using fraudulent or dishonest practices in the conduct of business.
Frederick Rogers, Jr., 2211 N. 16th St., Milwaukee, WI 53205-1217, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Kavon Stevenson, 3011 Westbrooke Dr., Cincinnati, OH 45238, was ordered to pay a forfeiture of $3,000.00. This action was taken based on allegations that he sold a Wisconsin resident unauthorized insurance, failed to respond to OCI's attempts to contact him regarding the sale, and failed to provide OCI with a current mailing address.

Jordan Zion, 955 S. Federal Highway, 4th Fl., Ft. Lauderdale, FL 33316, was ordered to pay a forfeiture of $9,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product, recommending the purchase of an unsuitable health insurance plan, enrolling consumers in health insurance plans without an appointment from the insurer, and failing to respond to OCI's requests for information.

Landolt Securities Inc., 900 W. North Shore Dr., Ste. 279, Lake Bluff, IL 60044, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to timely report administrative actions taken by another state and the Financial Industry Regulatory Authority.
OCMI Health & Benefits LLC, 225 E. Dania Beach Blvd., Ste. 216, Dania Beach, FL 33004, had its application for an intermediary firm license denied. This action was taken based on allegations of failing to disclose a criminal conviction on a previous licensing application, an administrative action by the Financial Industry Regulatory Authority, and administrative actions taken by Kansas and Florida.

Kaitlin Anderson, 1920 4th Ave., Baldwin, WI 54002, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that she committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
Theodore Hudson IV, 1608 Holmes Ave., Racine, WI 53403, was ordered to pay a forfeiture of $500.00. This action was taken based on evidence that he committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
Benjamin Mayo, 720 W. Mill St., Apt. N., Eagle River, WI 54521, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that he committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
William Ready, 307 Park Ave., Beaver Dam, WI 53916, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that he committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
Bertha Webb, 1382 Western Ave., Apt. 3, Green Bay, WI 54303, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that she committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
