Wisconsin releases enforcement actions for August
The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for August.
The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.
Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.
Allegations and Actions Against Agents
Omar Abdelrazek, 2020 E. Vandalia St., Broken Arrow, OK 74012-6166, agreed to the surrender of his Wisconsin non-resident insurance intermediary license. This action was taken based on allegations of unauthorized enrollments of consumers into health insurance plans.
Shawn S. Baldwin, N72W23656 Butler Dr., Sussex, WI 53089-3302, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Cody B. Bishop, 1065 Bridlewood Way, Brandon, FL 33511-6250, was ordered to pay a forfeiture of $3,000.00. This action was taken based on allegations of selling unauthorized insurance to Wisconsin consumers, misrepresenting to consumers that the unauthorized insurance was authorized in Wisconsin, failing to properly consider the suitability of the unauthorized insurance for the Wisconsin consumer, and for failing to respond to OCI's inquiries.
Frank Y. Cruz, 3221 Stockton St., Apt. 409, North Fort Myers, FL 33903-1601, agreed to pay a forfeiture of $500.00. This action was taken based on allegations of misrepresentation during the sale of an insurance product by the improper use of another agent's NPN in order to bypass appointment requirements and submitting insurance business without an appointment from the insurer.
David A. Degarmo, 2717 Omaha Dr., Janesville, WI 53546-4403, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Dynah R. Edang, 3772 E. Pulaski Ave., Cudahy, WI 53110-1927, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Eric Escalante, 9701 Yellow Stone Pl., Tampa, FL 33634-1093, had his Wisconsin non-resident insurance intermediary license revoked. This action was taken based on allegations of violating Wisconsin insurance regulations including failure to timely report an administrative action, failure to respond to or pay a previous forfeiture when due, and failure to update his contact information with OCI.
Angela M. Flemister, 5467 N. 57th St., Milwaukee, WI 53218-3228, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Ashlynn Hughes, 19506 Star Haven Dr., Cypress, TX 77433, agreed to surrender her Wisconsin non-resident insurance intermediary license. This action was taken based on allegations of failing to pay a previous forfeiture when due and failing to respond to OCI inquiries.
Stephanie A. Joyner, 3750 Kecoughtan Rd., Apt. 46, Hampton, VA 23669, agreed to voluntarily surrender her insurance intermediary license. This action is based on allegations of failing to timely disclose administrative actions taken by other states and violating a warning letter issued by OCI.
Janelle C. Levers, 15957 Vernon Way, Crivitz, WI 54114-7919, had her insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Amy Lichter, 2820 14th Ln., Kenosha, WI 53140, had her application for a Wisconsin insurance intermediary license denied. This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license, improperly misappropriating monies or property received in the course of doing insurance business and having a history of administrative actions.
Desiree Mann, 980 N. Federal Hwy., Ste. 308, Boca Raton, FL 33432, had her Wisconsin non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to timely disclose administrative actions taken by other states, providing incorrect information on a license application, and violating an OCI order.
Raymond Monk, 8440 N. Greenvale Rd., Milwaukee, WI 53217-2430, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Carl A. Payne, 2347 Charter Wood Ct., Maryland Heights, MO 63043-1405, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product.
Arturo A. Perez, 12985 SW 130th Ct., Unit 201, Miami, FL 33186-5346, agreed to pay a forfeiture of $6,000.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product without an appointment from the insurer.
Kayla Riggs, 2307 Lafayette Blvd., Fredericksburg, VA 22401, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.
Tamika Turman, 12124 Highway 22 E., New Site, AL 36256-3245, had her Wisconsin non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to respond to inquiries from OCI and failing to disclose her criminal history.
Cha Neng Vang, 5735 N. 95th St., Milwaukee, WI 53225-2611, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Patricia L. Walker, 12656 Southern Highlands Pkwy., Unit 1040, Las Vegas, NV 89141-3325, agreed to the surrender of her Wisconsin insurance intermediary license. This action was taken based on allegations of making misrepresentations in the sale of insurance products and recommending the purchase of unsuitable health insurance plans.
Damon Wallace, 3812 Chancery Pl., Fort Wayne, IN 46804, had his application for a Wisconsin non-resident insurance intermediary license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to insurance and failing to disclose all relevant information in his license application.

This action was taken based on allegations of having convictions substantially related to the circumstances of holding an insurance license, providing incorrect, misleading, incomplete, or materially untrue information on a license application, and failing to comply with an administrative or court order imposing a child support obligation.
Allegations and Actions Against Companies
Humana Insurance Company, 500 W. Main St., Louisville, KY 40202, was ordered to pay a forfeiture of $5,000.00. This action was taken based on allegations of making misrepresentations in the advertising of an insurance product.
Allegations and Actions Against Other Entities
Dawson Kollross, N5866 County Line Rd., Luxemburg, WI 54217, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that he committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
David Kugler, 557 Mary Knoll Ln., Watertown, WI 53098-1217, was ordered to pay a forfeiture of $1,000.00. This action was taken based on evidence that he committed a fraudulent insurance act by knowingly presenting false information for the purpose of receiving annuity payments.
Latoya Lewis, 17390 W. Crest Hill Dr., Apt. 206, Brookfield, WI 53045, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that she committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
International Health Sharing, Inc., d/b/a "Netwell", 12117 Beecave Rd., Ste. 202, Austin, TX 78738, and PO Box 343, Ringwood, NJ 07456, was ordered to cease and desist writing healthcare sharing ministry products in the State of Wisconsin. This action was taken based on allegations of doing the business of insurance without authority and making communications about its business that contain false and/or misleading information which make it difficult for consumers to distinguish it from traditional health insurance.
Shazad Enterprise LLC, d/b/a Gig's Petro Pantry, 2306 W. Fond du Lac Ave., Milwaukee, WI 53206, was ordered to pay a forfeiture of $1,500.00. This action was taken based on evidence that the entity perpetrated insurance fraud by knowingly presenting false information on their annual worker's compensation applications and audits.
