As part of the program, eligible Senior Care Plus members will have the opportunity to meet one-on-one with a participating pharmacist for a Comprehensive Medication Review (CMR). After discussing the member's complete medication regimen, the pharmacist will work with both the member and prescriber to address any medication-related concerns. In follow up to the appointment, the member will receive a Personal Medication List and Medication Action Plan outlining the key discussion points.
Through its Connect™ Platform, OutcomesMTM supplies participating pharmacies with alerts and information concerning patients' medication use patterns as well as guidance on working with patients and doctors to close key therapy gaps.
