The comprehensive population health management solution is focused on proactively managing care in behavioral health, long-term care, home health and hospice settings. The population health management offering supplements and extends health system population health programs by identifying community-based patients that require follow-up and ongoing monitoring.

The population health management solutions include a SaaS-based care management module that is integrated with the post-acute EHR, automatically monitoring and managing cohorts as a byproduct of care delivery. The offering provides an analytics engine that allows providers to further stratify and manage populations. The population health management solution captures data and quality metrics that can be used to track and improve outcomes to maximize incentive payments. The entire solution is built on the Netsmart interoperability network, making it easy to exchange and aggregate data with health systems and other community care providers.

Nearly 500 organizations use the Netsmart population health management solutions today to manage targeted populations in behavioral health. These solutions are now available to manage ACA health homes, transitions of care and special needs populations. The interoperability network has already established 1 million connections at more than 70,000 locations and serves as a foundation for full deployment of the solution.
