Product Specifications: The contractor shall provide a toolkit with all of the resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified.
The Evacuation Toolbox must contain the following supplies:
a. Evacuation Group Vests
b. Patient Evacuation Go-Pouches
c. Patient Information & Tracking Forms
d. Healthcare Evacuation Reference Guides
e. Wristbands, and more
f. Constructed from the best practices of emergency management evacuation procedures.
Additional Specifications: The toolbox shall support the Joint Commission standards for enhancing patient communication and shall align with the Hospital Incident Command System (HICS), which are as follows:
i. Assign the following Evacuation Incident Management positions on the unit/department:
" Evacuation Group Supervisor - Directs horizontal movement and patient holding areas. Coordinates vertical movement on a particular unit/department as directed by the Evacuation Branch Director
" Horizontal Movement Unit/Team Leader - Responsible for the movement of patients from their room/location to a designated holding area.
" Patient Holding Unit/Team Leader - Maintains continuity of care until patients are handed to a Vertical Movement Team.
ii. Distribute Evacuation Group Vests, Job Action Sheets, Healthcare Evacuation Reference Guides, Clipboards, and Pens.
iii. Assign staff to the following functions, as needed:
" Patient Mobility Assessment - Provide them with a Healthcare Evacuation Reference Guide
"
" Patient Holding Area(s) - Provide them with a Job Action Sheet
iv. In a non-emergent evacuation (time is available to mobilize staff & resources) await instructions by the Evacuation Branch Director before performing any patient movement activity

The contractor shall delivery the required toolkits, FOB Destination, to the address provided no later than 60 calendar days or less from date of award. Toolkits shall be delivered to:
