Amendment 1 RFQVA246-14-Q-0798 The purpose for Amendment 1 is:
a) Provide the answers to the technical questions
b) Attached a revised schedule.
TECHNICAL QUESTIONS
1. In the Schedule of Services document we are asked for the unit price & total. In the rows where we are our response is expressed as a discount (example: AWP-65%), should the # we enter in the total column be based on the average cost of the discounted Rx multiplied by the quantity
ANSWER: Provide the percentage, expressed as a percent in the column titled Unit Price (Percent Discount). A revised schedule document has been revised and attached (Schedule of Services VA246-14-Q-0798 Revised).
2. In the Special Contract Requirements document, there is a section that states: All contractor employees are subject to the same level of investigation as VA employees who have access to VA Sensitive Information. The level of background investigation commensurate with the level of access needed to perform the statement of work is:
ANSWER: The background investigation requirement is due to the nature of the requirements access to sensitive information, protected health information, and privacy protected information. The contractor that receives award of this contract will adhere to the background investigations as stated in the Special Contract Requirements.
3. Is a fully executed National Business Associate Agreement acceptable
ANSWER: Yes, a National BAA is negotiated and executed by the NDS/HIA Office and covers agreements between two or more VHA facilities (to include services contracted by a Veterans Integrated Systems Network (VISN), Regional or VHA Program Office contracting office) and a business associate is acceptable for use. No other BAA needs to be executed when a national BAA is in place without prior approval by the NDS/HIA Office.
4. What needs to be submitted I did not see a Standard Form 1449 to submit.
ANSWER: This was issued as a combined synopsis/solicitation procedure and the SF 1449 is not used for issuing the solicitation. Refer to the Combined Synopsis Solicitation section titled Instructions to Offerors for information about submitting a technical proposal.
5. I am hoping to find out if there is an incumbent contractor currently performing these services. If so, could I please find out the contractor name and contract number
