State Disciplines Health Care Providers

OLYMPIA, Wash., Aug. 13 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In June 2012 the Massage Program entered into an agreed order with Douglas L. Black (MA00011374) and placed his credential on probation for three years.

In June 2012 the Nursing Assistant Program charged registered nursing assistant Rebecca Ann Faye Melcher (NA60118406) with unprofessional conduct. Melcher allegedly tested positive for marijuana during a workplace drug screen.

King County

In June 2012 the Nursing Commission vacated the June 2012 summary suspension order and withdrew the statement of charges against registered nurse Patrick E. Ferrer (RN60032847).

In June 2012 the Emergency Medical Services and Trauma Systems Program charged emergency medical technician Paul M. Hermosillo (ES01172157) with unprofessional conduct. Hermosillo allegedly was in possession of cocaine and methamphetamine. He pleaded guilty to solicitation to possession of methamphetamine in 2012.

In June 2012 the Medical Commission charged physician David D. Herrington (MD00011353) with unprofessional conduct. He allegedly provided substandard care to an emergency room patient.

In June 2012 the Chemical Dependency Professional Program ended the probation order against chemical dependency professional trainee Stacy Lynne Holt (CO60117670).

In June 2012 the X-Ray Technician Program charged Randall Jay Kincheloe (XT00003179) with unprofessional conduct for failing to disclose, on a credential application, a disciplinary action taken against his other credential.

In June 2012 the Veterinary Board charged veterinary technician Aaron T. Scofield (AT60237131) with unprofessional conduct. Scofield allegedly diverted controlled substances from his workplace, altered patient records to cover it up, and falsified drug log entries. He was also criminally charged with third-degree theft for the conduct.

In June 2012 the Chiropractic Commission entered into an agreed order with chiropractor Lawrence P. Stanton (CH00002345) to reinstate Stanton's license and place it on probation for two years.

In June 2012 the Nursing Assistant Program charged registered and certified nursing assistant Teresa Margaret Trudeau (NA60134420, NC60052315) with unprofessional conduct. Trudeau allegedly violated a protection order by contacting and communicating with a patient.

Lewis County

In June 2012 the Nursing Assistant Program granted the credential of certified nursing assistant Rhonda Jo Baxter (NC10093371) and placed terms and conditions against her credential. Baxter was convicted for methamphetamine possession on multiple occasions from 2001 to 2010 and driving with a suspended license in 2011. Baxter was also convicted seven times for driving with a suspended or revoked license between 1999 and 2010.

In June 2012 the Nursing Commission charged licensed practical nurse Jill Holguin (LP60122511) with unprofessional conduct. Holguin allegedly failed to properly document administering or disposing controlled substances on multiple occasions.

Okanogan County

In June 2012 the Physical Therapy Board charged physical therapist Justin L. Schober (PT00008835) with unprofessional conduct. Schober allegedly failed to maintain appropriate boundaries with a minor patient, and engaged in an inappropriate relationship with the patient.

Pierce County

In June 2012 the Nursing Assistant Program charged certified nursing assistant Raul Sosa Corona (NC60143696) with unprofessional conduct. Corona allegedly didn't report a patient's injury at the facility where he worked.

In June 2012 the Dental Commission charged dentist Jin S. Miao (DE00007830) with unprofessional conduct. Miao allegedly provided substandard care while treating a minor patient.

In June 2012 the Agency Affiliated Counselor Program ended the probation order against Jerry Lee Watson, Jr. (CG60150513).

In June 2012 the Chiropractic Commission charged chiropractor Edward L. West (CH00001640) with unprofessional conduct. West allegedly billed insurance for services not provided and failed to keep proper patient records. He also allegedly asked patients to come to his office and sign back-dated attendance sheets stating they'd received treatments on certain days when they had not.

Snohomish County

In June 2012 the Medical Commission charged physician Ella Y. Bostanjian (MD60039274) with unprofessional conduct. Bostanjian allegedly failed to comply with a previous stipulation.

In June 2012 the Licensed Mental Health Counselor Program released Erik N. Bracht (LH00007519) from terms and conditions set against his credential.

In June 2012 the Nursing Assistant Program charged registered nursing assistant Bobbie Jean Scholle (NA60075713) with unprofessional conduct. Scholle was convicted of second-degree theft and third-degree assault in 2011.

Spokane County

In June 2012 the Pharmacy Board released pharmacist intern and pharmacist Kyle Shain Brown (IR00069473, PH60175806) from terms and conditions set against his licenses.

In June 2012 the Nursing Commission entered into an agreed order with registered nurse Catharine Jessica Farmer (RN00136035).Farmer must comply with terms and conditions set against her credential.

In June 2012 the Chemical Dependency Professional Program ended the probation order against chemical dependency professional trainee Anthony L. Williams (CO60142034).

In June 2012 the Nursing Commission charged registered nurse Kaarene D. Parker (RN00130561) with unprofessional conduct for failing to comply with a previous order.

Stevens County

In June 2012 the Health Care Assistant Program released Ronald William Beck (HC60248452) from terms and conditions set against his credential.

Thurston County

In June 2012 the Chemical Dependency Professional Program granted the credential of chemical dependency professional trainee Robin Louise Cannon (CO60212290) and placed it on probation for one year.

Walla Walla

In June 2012 the Nursing Commission reprimanded registered nurse James Corin McGraw (RN00166174) for sexual misconduct. McGraw sent numerous sexual texts to a patient's wife and made sexual comments to a patient.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
